GP Card for 2ww – Covid version

This is a new card sent out to all practices in printed form advising patients on what to expect from a two week wait referral. There are two copies, one highlighting the changes that Covid has occurred, and the other without the Covid information for the future.

Please note: The hospital’s Referral and Booking Service number has changed to 0113 5181824.
