COPD – Integrated pathway

NHS Leeds South & East CCG, on behalf of the city, is leading on a project to look at improving integration between the community and hospital respiratory specialist teams.

There have been significant reductions in the number of emergency admissions for COPD since work began on this pathway in 2003, however in recent years there has been a rising trend in admissions and the cost of those admissions.

An initial review of COPD services took place in 2013 and found that while there is some excellent COPD care currently being delivered in primary care, community and hospital settings in Leeds, the absence of an integrated care pathway approach based on these guidelines across all providers is compromising the optimisation of quality and effectiveness for service users and the whole system.
