NHS Leeds South and East CCG, on behalf of the city, is leading on a project to look at improving integration between the community and hospital respiratory specialist teams.
Since 2003, there have been significant reductions in the number of emergency admissions for Chronic Obstructive Pulmonary Disease (COPD). However, in recent years there has been a rising trend in admissions and the cost of those admissions.
An initial review of COPD services took place in 2013. This review tells us there is some excellent COPD care in Leeds. However, the absence of an integrated care pathway approach across all providers is compromising the optimisation of quality and effectiveness.
