The work stream is developed to support practices to contact at risk patients during the pandemic of COVID-19. Each PCN is encouraged to develop a mini team to support the operational development of the work stream.
The Primary Care Development Team can support PCNs with the following:
- Help each PCN coordinate their own mini team made up of HCA/ nurses, admin staff to proactively contact via sms or call the vulnerable list of patients proactively
- See which PCNs may need additional support of a person or people into the mini team to assist with calls and support – practices would need to grant access to their clinical system for any extra people working in the mini team to provide smart card access
- Making contacting with social prescribing (Linking Leeds) to align their role into helping in the min teams
- Making contact with named people representing local 3rd sector partners to pull on extra welfare support for any vulnerable patients needing wider support
